With regards to

What triggered this global trend? Which
other factors then accelerated this trend? (Notice the roles of the various
stakeholders in perpetuating/promoting such trends – the key stakeholders are
governments, corporations and the public)

§ Intellectual Property rights (whether governments have championed them) - this affects countries' access to medical technology/breakthroughs in medicine and the proliferation of such technology
